Older Adults Provider Survey Assets 7.5.23_Page_1

Advance OC is conducting the Orange County Older Adults Needs Assessment in collaboration with the County of Orange and the Office on Aging.

Paid Providers and Caregivers for older adults in Orange County are asked to complete a survey and share valuable insights from hands-on experience working with these individuals and understanding their needs.

The definition of Provider is anyone Paid to serve individuals 60 years and older, including Doctors/Physicians, Psychologists/Psychiatrists, Dentists, Pharmacists, Nurse Practitioners, Nurses, Social Workers, Nutritionists, Lab Technicians, Medical Assistants, Case Managers, Intake Specialists, Patient Navigators, Outreach Workers, Health Promoters, Cultural Representatives, Law Enforcement, Fire Service, Bus Operators, and City Staff.

The Older Adults Needs Assessment serves as a powerful tool to identify and prioritize the needs of older adults, ensuring that efforts are focused and responsive to the community’s real issues. The survey results will support the development of Orange County’s Master Plan for Aging.

The survey takes approximately 10-15 minutes to complete and closes on August 31, 2023.
